NYS Part 100 Regulations list the Home Instruction responsibilities of both the district and the parent. 

Parents are required to submit an Individualized Home Instruction Plan and Quarterly Reports.  The forms provided in the links on the right side of this page are templates that may be used, or parents may submit their own forms, as long as they contain the required information.
