Welcome to Roslyn Public Schools!
IMPORTANT INFORMATION:- Please read these instructions. They contain important information, including a listing of required documentation and forms.
- Children cannot be registered (and attend school within the Roslyn School District) unless the family is living within the Roslyn School District. A contract for purchase of a home is not considered proof of residency.
- Children need not attend the registration appointment.
- PRE-KINDERGARTEN Registration: The child must be 4 years of age on or before December 1. Admission into the Pre-K Program is subject to a lottery selection.
- KINDERGARTEN Registration for school year: Packets are mailed to families at end of January. If you do not receive a packet, please email Ms. Gina Polak at GPolak@roslynschools.org. Include your child's first and last names and "K fall (year)" in the subject line of the email. In the body of the email, list the following information: child's first and last names, date of birth, address and parent's phone number. The child must be 5 years of age on or before December 1.
- Steps to register your child/children:
- Gather the required documents and Roslyn School District forms (read the instructions on this webpage regarding required documents and forms).
- Create and complete an online account. Use the New Student Registration link to the left of this page. Create an online account and select a registration appointment. Appointment slots are added as needed.
- Create an in-person registration appointment through your online account.
- Bring the required documents and forms to the in-person appointment. (Forms available for download below) Notarized documents must be completed prior to the appointment. Roslyn School District does not provide a notary.

REGISTRATION INSTRUCTIONS
We are pleased to welcome you to the Roslyn Public Schools!
In order to safeguard the health of your youngster, to place your child in the most appropriate program, and to conform with New York State law and District Policy, we will need certain information and records.
Section 210.45 of the Penal Law of New York State prohibits the making of a punishable false written statement. False statements knowingly made are punishable by a Class A misdemeanor punishable by up to six months in prison or a fine up to $1000. The District will rely on your representation with respect to residency; you will be legally responsible for any inaccuracy or misrepresentation of the facts, including, but not limited to, THE PAYMENT OF TUITION AND ATTORNEY’S FEES.
***THE FOLLOWING DOCUMENTS AND FORMS ARE REQUIRED AT THE TIME OF YOUR APPOINTMENT:
- ORIGINAL OR CERTIFIED COPY of the Child’s BIRTH CERTIFICATE. (NOTE: The ORIGINAL OR CERTIFIED COPY of the Child’s Birth Certificate is the preferred documentation since it also satisfies the "Proof of parental relationship" requirement.) Although the ORIGINAL Passport is accepted, you must also provide the original marriage certificate or Order of Paternity to provide proof of the names of the parents.
- ORIGINAL Parents' Driver’s Licenses. If possible, bring a copy of the Driver's License of the parent who does not attend the registration meeting. Only one (1) parent (who is a resident of the Roslyn School District) needs to be present at the registration appointment.
- ORIGINAL Required NYS School Health Examination Form and list of Immunizations completed by a New York State Certified Physician. The physical examination must be recent (i.e., must take place within one (1) year of the start of school). See below for the Form and additional information.
- Proof of Residency (see below)
- Proof of parental relationship, if required (see below)
- Disclosure Statement: The form is located below. (This form requires a notary. Roslyn School District does not provide a notary.)
- Authorization for Transfer of Student Records: The form is located below.
- Home Language Questionnaire (2 pages): The form is located below.
- Health Office Registration: The form is located below.
- Computer Equipment Sign-out Form: The form is located below.
- Additional forms may be required. (If you rent, the Landlord/Owner Affidavit and the Renter/Non-Owner Affidavit may be required.) The forms are located below.
These items must be approved by Central Registration before your child may be registered.

PROOF OF RESIDENCY - REQUIRED DOCUMENTS
*** According to NY State Law, in order to register your child/children in the Roslyn Public Schools, you must be physically domiciled at your Roslyn address.***
Proof of residency: THREE (3) documents are required. Bills/Statements must be current (i.e., dated within three months):ANY ONE (1) OF THE FOLLOWING:
1) DEED OF PREMISES (ALL PAGES)
2) CURRENT MORTGAGE STATEMENT (LISTING ADDRESS BEING MORTGAGED.)
3) NASSAU COUNTY REAL ESTATE TAX BILL
4) HOUSING AGREEMENT (ALL SIGNATURES MUST BE NOTARIZED)
5) ORIGINAL CURRENT NOTARIZED LEASE**PLUS ANY TWO (2) OF THE FOLLOWING:
1) PAID MOVING COMPANY STATEMENT WITH ROSLYN ADDRESS AND DATE OF MOVE
2) CURRENT FUEL DELIVERY STATEMENT
3) CURRENT PSEG OR NATIONAL GRID STATEMENT
4) CURRENT ROSLYN WATER DISTRICT STATEMENT (FULL STATEMENT - INCLUDING THE PAYMENT STUB)
5) CURRENT CABLE STATEMENT (LISTING LOCATION OF SERVICE)
6) CURRENT CAR INSURANCE OR REGISTRATION WITH ROSLYN ADDRESS
7) CURRENT HOMEOWNER’S INSURANCE POLICY or CURRENT RENTER’S INSURANCE POLICY. (FULL POLICY MUST BE PROVIDED - NOT JUST THE BILL/STATEMENT.)** IF THE SIGNATURES OF BOTH THE LANDLORD AND THE RENTER ARE NOT PROPERLY NOTARIZED ON THE LEASE, BOTH THE LANDLORD AND THE RENTER MUST PROVIDE AFFIDAVITS, AS WELL AS SUPPORTING PROOF THAT THE PROSPECTIVE STUDENT AND PARENT(S) ARE RESIDING IN THE ROSLYN SCHOOL DISTRICT. YOU CAN FIND THE LANDLORD/OWNER AND RENTER/NON-OWNER AFFIDAVITS BELOW.
NOTE: A SIGNED CONTRACT OF SALE/PURCHASE IS NOT ACCEPTED AS PROOF OF HOME OWNERSHIP.
REGISTRANTS WHO CANNOT PROVIDE THE ABOVE ITEMS MAY BE REQUIRED TO SUBMIT ADDITIONAL DOCUMENTATION TO VERIFY RESIDENCY OR MAY BE REQUIRED TO SIGN A SWORN AFFIDAVIT. EXCEPTIONS WILL BE MADE WHERE APPROPRIATE. -
PROOF OF DATE OF BIRTH - REQUIRED DOCUMENTS
- Students will be eligible for registration when the parent or legal guardian presents an original OR certified copy of the birth certificate with a raised seal. (The original/certified copy will be returned to you during the appointment.) Although the ORIGINAL Passport is accepted, you must then also provide the original marriage certificate or Order of Paternity to provide proof of the names of the parents. The ORIGINAL OR CERTIFIED COPY of the document will be copied and then returned to the parent during the registration appointment.
- State law requires that the child’s legal name must appear on the permanent record card, health card, transcripts and all diplomas as well as any other official records.
- Students will be eligible for registration when the parent or legal guardian presents an original OR certified copy of the birth certificate with a raised seal. (The original/certified copy will be returned to you during the appointment.) Although the ORIGINAL Passport is accepted, you must then also provide the original marriage certificate or Order of Paternity to provide proof of the names of the parents. The ORIGINAL OR CERTIFIED COPY of the document will be copied and then returned to the parent during the registration appointment.

PROOF OF PARENTAL RELATIONSHIPS - REQUIRED DOCUMENTS
STUDENTS WILL BE REGISTERED WHEN PROPER PROOF OF PARENTAL RELATIONSHIP IS PRESENTED.
Providing an ORIGINAL or CERTIFIED COPY of the BIRTH CERTIFICATE listing the parents' names is sufficient proof of the parental relationship. (The ORIGINAL or CERTIFIED COPY will be copied and then returned to the parent during the registration appointment.)
A child must reside with his/her parent(s) or person(s) in parental relation. To establish proof of parental relation, in lieu of the ORIGINAL OR CERTIFIED COPY of the BIRTH CERTIFICATE, the individual registering the child must submit documentation, which can be an affidavit, indicating either (1) that they are the parent(s) with whom the child lawfully resides; or (2) that they are the person(s) in parental relation to the child, over whom they have total and permanent custody and control, and describing how they obtained total and permanent custody and control, whether through guardianship or otherwise. Additional documentation may be requested.
- Divorced Parents
*The District will not restrict access to a child by the non-custodial parent unless an appropriate court order is provided. [Originals or notarized copies]
- Foster Parents
*Placement Order Form DSS 2999 is required.
- Guardianship
*Guardianship documents signed by a court officer are required.

PROOF OF ANNUAL PHYSICAL EXAM AND IMMUNIZATIONS - REQUIRED DOCUMENTS
IN ADDITION TO THE NYS SCHOOL HEALTH EXAMINATION FORM SIGNED BY A NEW YORK STATE CERTIFIED PHYSICIAN, PROOF OF ALL IMMUNIZATIONS MUST BE SUBMITTED. ONE OF THE FOLLOWING PROOFS MAY BE SUBMITTED ATTACHED TO THE IMMUNIZATION FORM:
- Certificate of immunization signed by a New York State licensed physician
- Certificate of immunization signed by a representative of an official health clinic
- If the NYS School Health Examination form signed by a NYS certified physician is not provided, the District’s physician may provide the physical examination and signed form
