Requesting Forms 1095-C and 1095-B

In accordance with IRS Notice 2025-15 and the Paperwork Burden Reduction Act, Roslyn Public Schools will provide Forms 1095-C and 1095-B only upon request. Employees who require a copy of their form should email 1095Request@roslynschools.org to submit their request.

Forms will be provided within 30 days of the request or by January 31 of the following year, whichever is earlier.
